"Any ol' DS-3025 may not be acceptable. They'll give you a DS-3025 whether you got the required shots needed for AOS or not. Some doctors fail to sign them. Some fail to complete the results section. I will say Knightsbridge clinic is very good at ticking the right boxes and signing if you got your shots and told them you had chickenpox.

There are certain things your DS-3025 should have to be considered complete.

Look at your DS-3025. If it has the following items, you don't need an I-693.

1. Shots marked (at least one of a series)

MMR

Td or Tdap or DT or DTP or DtaP (date no longer than 10 years ago)

Varicella or VH written by it if you had chickenpox

2. Everything else marked not age appropriate or medically appropriate.

3. RESULTS section filled in with "incomplete". AND "may be eligible for blanket waiver"

4. Signed and dated by the doctor

If it's not completed like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025.

Do NOT do what UK person ECWilloughby did recently and send in an I-693 without a doctor's part filled in along with the DS-3025. Do one form or the other.

The USCIS is supposed to have your medical files, but sometimes lose them or fail to match them up with your AOS. Do not get another exam unless you get an RFE that says they do not have results of your medical. It means they lost them, but you are probably stuck with paying for a new exam. Your official vaccination results are also part of that file. We send the DS-3025 photocopy more to remind the person opening the mail that we are intending to use that DS3025 to prove our vaccinations, rather than an I-693. The official "sealed" version is in you A-file (brown envelope) which they are supposed to get and match up with your AOS.

The DS3025 has to have all those things, not just some of them. Incomplete is fine if the box about blanket waiver is also ticked"

7.31.2011- POE (BWI)-- NEVER ISSUED AN I-94 (WTH)

8.15.2011- I-94 Obtained from IAD's CBP Inspection Office

AOS & EAD

9.8.2011- Mailed USPS Priority Express

11.29.11- RFE for Income Tax Return-- SEND YOUR ENTIRE TAX RETURN DOCUMENT!!

11.30.11- mailed Tax Return UPS Priority Express

12.9.11 AOS "transferred to a USCIS office" - - supposedly our local one

12.14.11- "touched"

12.15.11- received EAD

2.8.12- put in a service request for GC

2.22.12- My husband was contacted by the local USCIS office here in Washington DC and told that it was infact transferred to the CSC. Not their office. She also suggested he keep checking on it because sometimes things just sit there and take up to 3 years to be processed--- SAYWHAT?! ... stay tuned

3.29.12- Approved, Green Card in production

4.4.12- Green Card received- NO INTERVIEW
