ANYONE IN CHICAGO ILLINOIS HAD THEIR CIVIL SURGEON DONE ALREADY?

Timeline

HELLO, everyone, anybody here from chicago illinois had their civil surgeon done already? and anyone here could advice me where and whats the doctors name?..coz i just had my appointment with the civil surgeon this morning and shes also accrideted with the USCIS but the thing is,,it seems like they dont know what i needed...i showed my vaccinaation report and CD from st. lukes but still they cant perform the I-693 for me because they need the whole report from St. lukes which,,we K1 dont have a copy to all of those medical results because it was all submitted to the POE already...i dont know which doctor to go anymore, that knows about immigration medical thing...

anyone here could share their experience here in chicago, illinois?

Explain to them that you need your DS-3025 to be transcribed in the I-693 because you just need the vaccination supplement to be submitted with your Green Card application. If in case they told you that you need another medical exam, show them page 4, #8 of the I-485 instruction and tell them that you don't need another medical exam because you had it in the Philippines.

I live in Chesterton Indiana, about 40 miles away from Chicago.

Hey I live in Illinois too! If you're in downtown try calling first the civil surgeons listed in the USCIS website (the one posted above this). They are so many and ask what they want and compare how much they charge too.

11/1/11 - AOS package Delivered

11/7/11 - NOA Received in the mail

11/12/11 - Biometrics Appointment received

11/28/11 - Case transferred to CSC

12/7/11 - Biometrics done

1/2/12 - EAD and AP approved (Card production)

1/11/12 - EAD/AP Card received

1/20/12 - Got Driver's License!

3/7/12 - GC approved!

3/9/12 - Got a job!

3/11/12 - GC on hand

That's right! Ask first how much they're going to charge you. There are too many of them in Chicago.

With mine, I paid $10 and I found the civil surgeon in Munster, IN which is 26 miles Southeast of Chicago... and for sure, CS from Chicago area charges more expensive than civil surgeons from other cities/states. I think $30-$50 is acceptable because the CS will just transcribe your vaccination sheet from St. Luke's and might or might not ask you to have another set of vaccinations.
