Question on DS-156 supplement sheet

Hello everyone! Interview upcoming next week and starting to get uber-stressed over here...

I was checking everything again and I found out there is an example form for the DS-156 at http://www.visajourney.com/examples/Form-DS-156.pdf

The example is very clear, except from the page 3: indeed I do have to add an additional sheet for the same question showed on the example, but all I did was adding a simple sheet signed by myself, I did not find anything similar to what is on the page 3 of the example linked up here, which have even a bar code.. :unsure:

Does anyone know anything about it? Thanks in advance and good luck to everyone :thumbs:
