You cannot go through this process by asking people here to guide you through every little step that you have to take. It's ok to ask questions to clarify issues and confirm things you are not 100% sure about, but it seems like you are not really willing to go through any effort to figure these things out yourself.

If you put "AOS cover page" in the search function of VJ, you'll find a ton of examples. If you put the same words in google you'll also get examples. All the cover page needs to entail is a clear list of the documents and items you are sending, as a courtesy for the IO who goes through your package. It is not a mandatory part of the package, so even if you did not include a cover page, you'd be fine - though it is good to have one.

I would kindly suggest that you go through some effort to figure out these things first yourself, and then if you can't find the answer, post a question here. I am only saying this because if you continue going through this process like this, you are bound to make mistakes with your forms and paperwork, which might substantially delay your AOS process.

Hello Visa Journey Members,

I come again asking questions.

My spouse does not have a middle name, she was not given one at birth and never was, and the forms ask for middle name, and we want to know what we would put in the middle name section for her?

Do we just leave it blank? or type none? or type N/A?

many thanks for taking the time to read and answer

I left it blank on our i-129F.

At the risk of being embarrassed I will disagree with Harpa Timsah (respectfully) I would say N/A incorrect. N/A means not applicable. Putting N/A would indicate that the middle name does not apply to your spouse when in fact she just doesn't have one. I think "None" or leave it blank is more appropriate.

Harpa please be gentle.

Be gentle... lol :) You are right that N/A means "not applicable" and the answer is in fact "none." However, I think N/A is good in this case because it is extremely obvious that N/A is not a middle name (because it includes a symbol), whereas there is the remotest possibility that none could be taken for a middle name.

All in all, they will look at the passport and derive the name from that, so I think anything is fine... N/A... none..."none"... and Pushbrk likes to recommend "--------" ; I really think all will get the job done.

I just answered the OP in short form because he has posted in excess of 10 topics in the past two days :)

AOS for my husband

When I was in the Marine Corps I heard a story of a young Marine who's parents named him R B Jones. Just letters that didn't stand for anything. He filled out his enlistment papers "Jones R(only) B(only)". His ID card had his name as Ronly Bonly Jones.

well, yes I have, I like finding answers in people's own words because it's easiest to comprehend and just easier overall because I don't have to look at this or that trying to find it, I can just look at the easy to comprehend answers.

If I may interject, another option for not having a middle name is to put dashes all the way through that field.

Hello Visa Journey Members,

I am writing yet again to ask about something I am not finding any good information on my own.

I know photographs are a neccesary thing to have. photographs of me and my newlywed wife together, but what about videos?

Could I put a video on my PSP and play a video of us in the past that we took of us together?

or are videos just a no go?

any information and knowledge regarding this would be greatly appreciated and thank you for taking the time to read and answer :)
