AFFIDAVIT OF A FRIEND question

I just have a quick question.

I will be using the format for Affidavit of a friend which is available here in VJ. At the lowest part of the template, it says....

" Sworn to and subscribed before me this _____ day of __________, 2011, by BUDDY O'MINE who personally appeared before me, is personally known to me or produced _____________________________ as identification, and did take an oath."

I don't know what that colored text means... can somebody please educate me on this. thank you!

The blank should contain a form of ID (e.g. Passport, Driver's License, etc.) to be presented to the Notary Public. This is to check on the identity of the person doing the oath.
