Medical Form DS2053 or DS2054

We just made our appointment for the medical. There are two forms for this, the 2053 and the 2054. From what I can find online, embassies in certain countries use the 2054 and some the 2053. I've seen some threads where people were sent back to the doctor to transcribe the info from one form to the other. Does anyone have the list of which countries use which forms?

Does anybody know about Thailand? The hospital told us to bring the 2053, but the embassy has the 2054 on its website.

We used the 2053 and the visa was jsut approved by the bangkok embassy in April. Unless, they just changed it between April and now, I would go with the 2053. Good luck!

I suggest try doing that. Just make sure it is the most updated forms.

I went with Pui back in April to BNH and we got the the forms from the hospital. When she had her interview in June she was given a yellow ship telling her to take her sealed medical records back and return with the sealed medical records with the updated forms. Once Pui returned with the updated documents she was given her visa. It happened to another lady who interviewed that day and she had gone to the other hospital in Bangkok. It also happened to a member who went to BNH around the same time we did. All of them got their visas. What ever happens just make sure you have the updated forms. If you have time maybe try emailing the embassy asking for a link to the most recent version of the medical forms. Just trying to make sure what happened to us does not happen to you. Good luck

I had my health check last month at Bumrungrad Hospital, I used 2054 (expiration date Jul-31-2013), 2053 has expired since Jul-31-2010.

I made appointment with Dr.Watcharapong, he's very used to K/CR applicants (most of the applicants from usvisa4thai.com went for him).

Thanks for the tips. We made appointment at Bumrungrad and the receptionist told us to bring the 2053. It's the expired one. Two emails to the embassy requesting clarification have gone unanswered. ...Is that unusual?

At this point I will bring both forms and see what the doctor says. They must do a lot of these and will know if the embassy is sending people back to have them transcribed.

There was a case someone posted awhile back where they used 2053 and they got sent back to do a 2054. I wouldn't use an expired form unless they refuse to fill it out.
