Help with figuring out Canadian website for Pkt. 3

We got Packet 3. However the instructions are vague. It says that my fiance needs to choose "permanently immigrate to the US" but there are several options within that selection. And what should we do after that? Just figure out which documents to send and send them in?

Hi! I found a few links for you! The 1st one is from the government website with a list of forms needed. Note that they say it is differnt for Montreal:

Then go here for Montreal and do exacty what is says:

http://montreal.usconsulate.gov/content/content.asp?section=visas&document=visa_pkt3

Make sure you fill out all these forms:

DS-230 part I and part II – 1 copy

DS-156 – 2 copies

DS-156K – 1 copy

Form DS-160 – 1 copy

Also follow the checklist of other documents you need such as the I-134 affidavit of support, pictures, birth certificate, etc.
