Hi Sis, I came in the US with my daughter. The idea is all the documents that you need are always x2 except the police clearance, certificate of no marriage and nbi but the rest will always be YOU and HIM.

-passport

-authenticated birth certificate

-medical exam

-DS forms

-visa payments

MY daughter's BC has her father signed on the back, when i was preparing our requirements, I called DSWD so i can get a her travel clearance but they said since she is below, legally mom's custody. You can send me message if you need my help :) good luck on your journey.

Actually in some countries a minor child can be appended into the parental passport. If this is the case then both visas are placed in the one passport.

The US Department of State recommends each to have there own passport

I agree with them. Having the youngest in Alla's passport turned out to be a headache within a year. He could ONLY travel with her and sometimes the plans didn't work that way and we wanted him to travel with me. Again check the laws in your country.
