Lost I-129S form

Hello,

I came to US on L1-B Blanket visa last year and at Port of Entry immigration officer kept my only copy of I-129S form. I have got another copy of my I-129S form from my employer but it is not signed/stamped by consulate.

Now, I am planning to get married very soon in India and would like to apply L2 visa for my wife. Please tell whether my (primary applicant's) I-129S form is a mandatory form to apply L2 visa for spouse or not. If yes, please tell the procedure to get it again signed by consulate (Can it be done from US because I am here?).

Also, I already contacted Port of Entry officers to get my I-129S form back but they were not able to help.
