Advance Medical in SLEC

First, for those of you who did an advance medical, what did you show? DS3032, AOS or IV bill with NVC case number? I already mailed out AOS and IV documents and I am sending my daughter for advance medical without an appointment letter. I just want to make sure that SLEC still let applicants do their medicals even without it. Second, my sister will accompany my daughter (she's 3 years old)to her medical, do I or my husband (petitioner) have to send an authorization letter?

NATURALIZATION
January 09, 2013-- sent N-400
January 28, 2013-- Biometrics

May 22, 2013-- Interview Passed- Officially a US Citizen!

St. Luke's guide/website Here is the website with more than you probably wish to know. Best of luck and I'd maybe call in advance to make for sure but I'd bet that your daughter will need at minimum an authorization from you if they permit your sister to bring her. I'm afraid they may be paranoid about the child being who she says she is........

HI,

Before my wife had a interview appointment I emailed St. Luke's and ask them if it was OK to get her medical done before her interview was scheduled, they responded with........

"Normally, appointment letter is presented to us during the medical examination. However it is your discretion to have the medical examination without the appointment letter, for as long as you know your case number and visa preference category.

For other information and requirements, kindly visit our website at <http://www.slec.ph/>www.slec.ph"

My wife went prior to her having a interview appointment and brought with her a valid passport, NOA2 with MNL number on it, filled out online registration form from St. Luke's website, IV bill and the email they responded back to me with. It was no problem and the only thing they said was it was valid for three months and to make sure that her interview is within three months or she'd have to re-do her medical.
