Thanks everyone for the great responses so far!

I have some additional questions about vaccines, and about the Civil Surgeon signing the I-693.

I keep trying to get a handle on exactly which vaccinations are "required" for AOS, and there seems to be differing information out there:

Fatima is 50 years old -- some tables of vaccinations break ages down ages at 18-64 years old, others more speficially (i.e. 18-26, 27-49, 50-59, 60-64)

So far it seems like she needs Td/Tdap, MMR (although may not be needed if over 50?), Varicella (unless can prove already immune?), and Influenza (although if not flu season, may not need it right then?). One chart we saw included polio vaccine!

Anyone have any definitive answers?

Then, once the Civil Surgeon gets the I-693, solely to document the vaccinations (since no medical exam required) -- do they cross out everything that is 'not applicable' on the form? What about when they sign the Civil Surgeon's Certification (part 5) that says they completed an examination? Do they cross out language that is 'not applicable' there as well?

Seems like we found a Civil Surgeon in Portland that charges a reasonable fee for signing the form, but we may have to 'walk him through' doing the form like this!!

You will see the table lists shots that are not in the USCIS list. That is because the table isn't JUST for USCIS so you need to compare the "USCIS required shots" to the list from USCIS (top link) and ignore the ones that USCIS doesn't require.

Based on this list and your advice that Fatima is over 50 then she will need:

- Varicella (or varicella history. She need only TELL them she's had varicella.. which is chicken pox).

- tDap

- Flu (if in season)

You're right that she's too old for MMR now so she shouldn't need it.

Beginning December 14, 2009, all applicants seeking adjustment of status or another immigration benefit that requires admissibility under section 212(a)(1)(A)(ii) of the Act must show proof of having received the following vaccines: mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, influenza, hepatitis B, haemophilius influenza type B, varicella, pneumococcal, rotavirus, hepatitis A, and meningococcal. HPV and zoster will no longer be required. I693, find a civil suregon who is approved by USCIS, they know what to do and what is required, from my experience. No matter what shots you get and approval to come on K1 visa, you will need to see a US civil surgeon in USA and get a review plus, additional TB check and review of the chest x-ray. If your exam was over 30 days- maybe more. Bring your shot records with you and chest x-ray to us in a separate item from the immigration package (closed). A real MD in USA will thoroughly review, take blood etc., their license and credibility is on the line. Hope this helps. See the VJ guides as well as government information. I paid separate for DR's paperwork for AOS.

I'll address this here.

For the "vaccination transcription" you complete part one. Then you (or rather the CS) ignores ALL of part 2 except the vaccination table. When it gets to the vaccination table all shots need to be marked either with a date, VH for "varicella history" (varicella only obviously) or one of the options from the "waiver" section which is the last 4 columns.

Going down the list you have:

1. DT - REQUIRED one of these or one of #2 needs to be done

1. DTP - REQUIRED one of these or one of #2 needs to be done

1. DTaP - REQUIRED one of these or one of #2 needs to be done

2. Td - REQUIRED one of these or one of #1 needs to be done

2. Tdap - REQUIRED one of these or one of #1 needs to be done

3. OPV (Polio) - Not age appropriate

3. IPV (Polio) - Not age appropriate

4. MMR - Not age appropriate

5. Hib - Not age appropriate

6. Hep B - Not age appropriate

7. Varicella (Chicken Pox) - REQUIRED either VH or a date for the first of two shots

8. Pneumococcal - Not age appropriate

9. Influenza - will depend on the season

10. Rotavirus - Not age appropriate

11. Hep A - Not age appropriate

12. Meningacoccal - Not age appropriate

Bearing in mind some of those "not age appropriate" she may have had as a child so she can put the dates of those shots in there (or the CS will).

Honestly though this is something CS should know. They ARE a CS from the USCIS website right??

Nope sorry. WRONG.

If you come on a K1 and you've had all required shots then you don't need to see a CS.

If you come on a K1 and you need further shots you take the DS-3025 to a CS who does a vaccination transcription ONLY. There is NO new TB test required, and no x-ray and no new medical required.

The chest x-ray need not come to the US (from your K1 medical) but they recommend you do simply for your own personal (not immigration related) medical records.

If you come on a K1 and you need further shots you take the DS-3025 to a CS who does a vaccination transcription ONLY.

OR... If you come on a K-1 and you've had all required shots, but you had them done some place other than at the medical (eg: your doctor), you take evidence of your shots and the DS-3025 to a CS who does a vaccination transcript only.
