can anyone please tell me why i got this letter from uscis saying that i need to submit a properly completed and signed Form I-693.

this is what the letter looks like

This office is unable to complete the processing of your Application to Register Permanent Residence or Adjust Status,

(Form I-485) without initial information. Submit ALL the information requested blow at one time. Failure to do so will

result in a denial. You must submit the information within 87 (eighty-seven) days to the following address: U.S.

Department of Homeland Security, PO Box 648004, Lee's Submit, MO 64002 Include this letter with your response.

The Service will process your form I-765, Application for Employment Authorization (if submitted) within 90 of the

receipt of your missing initial evidence. This is pursuant to Title 8, Code of Federal Regulations, Section 103.2(b)(10)(ii).

Submit a properly completed and signed Form I-693,Report of Medical Examination and Vaccination Record for the applicant

on Form I-485 (all pages of Form I-693 must be the same revision date). The Medical examination must be conducted by a civil

surgeon who has bee designated by USCIS.The medical must be issued less than one year prior to the filing date of Form I-485

Medical examination using the 06/05/08 revision date, the medical must be conducted before 12/31/09.

Medical examination using the 10/14/09 revision date, the medical must be conducted between 10/14/09 and 07/2110.

Medical examination using the 02/25/10 revision date, the medical must be conducted on or after 02/25/10.

Medical examination using the 07/20/10 revision date, the medical must be conducted on or after 07/20/10.

The civil surgeon must conduct the exam as directed in the the instruction form I-693 and the centers for disease Control's

Technical instructions for the Medical Examination of aliens in the United States. Both the medical examination and the

vaccination record must be completed when required.

A)Is it because the civil surgeon didn't complete the form rite?

B)Should i have it done at another CS?

C)And how should i go about doing it?

We don't know what you sent in. Did you go to an authorized civil surgeon and not just a doctor?

Thanks for the reply, so i send in Forms i-485 together with i-765 and i-864. When i got the Medical done

i got the sealed envelope together with the i-693 form. The i-130 is already approved and they already

send me a notification date for my finger printing.

I only got the medical done on 07/05/2011 and i submitted everything on 07/12/2011.

And yeah i did get it done at authorized civil surgeon. Do u think that the civil surgeon was the one how mess up?

What should i do now?

Should i get it done at another authorized civil surgeon?

And is it gonna cost as much as it did the first time?

Hm..

Maybe the civil surgeon did not properly sign it, fill it in, etc.

When I went for my appointment, they brought me the paperwork, had me double check my name, address, etc and then had me sign it. Did they have you sign it?

They also gave me a copy of the paperwork for my records. Did you receive one?

One thing you could do is go back to the civil surgeon, bring this letter with you and ask if there was something that possibly went wrong and if he could re-administer the test. He should have that paperwork in his records. That way, if he has to do it again, there's no need to do the vaccinations since he knows you've had it done...

When i went to my CS I noticed that they had an old form. it sounds like this might be your issue. They should have given you a copy, check what is missing. Are the pages from the same document? Does it have a barcode at the bottom of each page? Did you remember to put it in the packet? (Sounds like a dumb qn but it's happened before).
