Hi, we intend to file on September 2nd but plan to travel out of the country on September 3rd, should I include that trip even though it's in the future?

Also, when I've sent documents in the past (for say Adjustment of Status) I've included a cover letter listing all the documents I'm attaching; in reading the instructions I don't see the suggestion for a cover letter. Please let me know if you are a previous filer if you've included one.

Can't remember if I did with the three AOS applications...

No, you can't include a future trip in the application. You will be requested to bring your travel documents to the interview, and all the changes that occurred between filing and interview should be explained/proved then. If it is a short trip and it's been over 3 years since you became a LPR by the time of the Oath, it will not affect your application.

No, you can't include a future trip in the application. You don't need to include the future trip, however, during the interview, the IO will ask you about the trips you have made in the past (in the event you made any after filing) and you can mention that last trip so he or she can ammend your N-400.

And oh, I put cover letter on mine, to make it more formal, I guess.

What if your travel plans change? I guess this is the question that I would consider the answer to your question... probably it will work either way. However, it may be smarter to update your file when you meet your IO as others have recommended. Travel outside the US is not frowned upon, but it is important to verify eligibility for physical presence and continuous residence.

If you post it on one day and travel the next then don't worry. If at the interview they ask something then tell the officer of your trip/s since the date of your application.
