So while my initial journey started off smooth, all is not well anymore. I have a couple of questions. From the time the visa is issued, how long does the visa holder have to enter the US and have anyone ever had it extended, or is it even possible?

The visa is valid for 6 months. I haven't heard anything about the extension of a visa. There will be a way but I am not aware of any thing like that.

6 months is the standard; sometimes it is less, especially if you've had the medical early.

You cannot extend the visa, but it can sometimes be re-issued. There would need to be very good reasons for that, such as a gravely ill close family member the beneficiary needs to care for, or children who want to finish out the school year in their home country before immigrating.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.
