Questions About I-751 When Divorced

Hello fellow members,

I would appreciate some help from you guys in regards to filling out the form I-751 when being divorced. I have a couple of questions regarding what to enter in certain parts of the form, so any help/answers would be greatly appreciated.

I just got divorced very recently and will be filing individually, so i have a few questions,

1. Part 3, #9 - Asks, Have you resided at any other addresses since you became a premanent resident?

When i first arrived here we lived at one location together, then just over a year into the maraige we moved into another location together with a roommate. Then when we ended up seperating i moved out to a friends house where i am currently, my question is, do i need to list all these 3 locations on a piece of paper? The 2 locations where we lived together and the location i am currently?

2. Part 4 - Information about the spouse whom you gained your conditional residence

Just one question here, it asks for the family name, i came here through my now ex wife who is the American Citizen, so what would i put in the family name box? would i put her maiden name or the name she obtained through marrying me?

3. Part 7 - Signature

There is a box open for the spouse to sign but since i am filing individually does this box remain blank or do i need her to sign this form or is it left blank?

well actually I have just been reading all over this today myself and you would leave the signature of her blank as you are ticking the box filing single due to--------- You tick box (D)

Yes you have to write on paper all the addresses you have lived at since being here in the US it clearly says that.

You would put here name before she married you as that is what would be o the petition 129f

1. Write down anywhere YOU have lived. You have had 3 addresses.

2. What is her name right now? Does she continue to use your name after divorce? Use her current name.

3. She does not sign because it is not a joint application. Put N/A
