A great marriage is not when the "perfect couple" comes together.

It is when an imperfect couple learns to enjoy their differences.

The is an actual form to submit (a civil surgeon in the US has them)...

Appendix A. Supplemental Form To I-693

"The fact that we are here today to debate raising America’s debt limit is a sign of leadership failure. It is a sign that the U.S. Government can’t pay its own bills. It is a sign that we now depend on ongoing financial assistance from foreign countries to finance our Government’s reckless fiscal policies."

Senate Floor Speech on Public Debt

why can't someone just tell me what to send along instead of the link? That would be a better and fast way to answer to my question becos I know that so many people go thru this route of adjusting and could help me with this faster than what am getting right now.

With the amount of time you've spent complaining about not receiving the "proper" help, you could have researched your own question with the links provided and gotten your answer.

in your country when you had your medical and vaccination done, the hospital will give you a copy of a vaccination sheet that you have to bring to the US with you...medical result will be directly submitted to the embassy in your country where youll have your interview, the embassy will send it to you together with your visa (enclosed in the envelope that you cant open<<<----for immigration officers only, they'll get that pocket from you in the immigration check in your port of entry to the US<<----airport). when you apply for the aos, together with all your requirements, send them a copy of your vaccination sheet which was given to you by the hospital where you had your medical back in your country...Note: the medical you had in your country should be not less than a year...i hope this helped...Good luck with you application
