Hi,

I have a question about having my husbands vaccination record from Morocco being transcribed onto the I-693 by a Civil Surgeon here. I have looked at the USCIS site to try to find doctors who can do this and they all seem to be near the USCIS local offices... which are not near me. I am in San Luis Obispo California and all of the service centers are about a 3 hour drive just to get there. Have people in the past had to drive that far just to get this done? Can it be mailed? We don't need the exam just the vaccination records done on the right form.

Does anyone have any advice as to how to find a Civil Surgeon in my area as I have yet to find anything.. and also does that sound reasonable to have to drive that far just to get that done?

That seems very odd that there aren't that many Civil Surgeons in your area. When I looked up CS in the Seattle area for daughter, there were so many to choose from.

The CS we went through allowed us to fax in the Vaccination Worksheet (from the medical exam for my daughter's K-2 Visa) to their office and then they transferred the information onto the Vaccination Supplement. If we would have been prepared to send in cash for the $35.00 fee (they only took cash) and a self-addressed stamped envelope, we wouldn't have even had to go into their office. But, fortunately, we were able to pick it up at the CS office ourselves.

Maybe you could call around to a few and see if they would be willing to do the same thing for you. We paid $35.00 for them to transfer the information onto the Supplement, and no additional vaccinations were required.

This is the only instance where I've heard of getting the I-693A by mail.

This thread would indicate that they no longer do it though. You might want to call to double check.

Ok I think I found someone who is only a half hour away which is better than a 6 hour drive to and from to have this done. I explained that he doesn't need the exam just the paper so she said yes we can do this for 10 dollars! YAY!

I hope that when I go take it to her that what she said will still be true.

San Luis is a verrrryyy small town and doesn't have much- especially no civil surgeons I guess which does seem odd but it will just be a short drive.

For what it's worth, although don't use this post as the facts, i'm just going on my experience....

When I had my K1 medical, I wasn't able to produce documentary proof regarding vaccinations. During the medical, I said i'd wait and get the vaccinations at a later date.I didn't have enough time to get them done in the UK between the medical and K1 interview, so left them until I was in the US. (I flew back from the US the day before my medical, and flew out a few days after the K1 interview)

I had the vaccinations done by our US family doctor, and he wrote a certified script saying what I had done.

My AOS paperwork was submitted without the vaccination suppliment, and I was never asked for it at my recent AOS interview. I was approved two days after, and never received an RFE for the form.

I guess some offices don't ask for them, and some do. I'm also thinking that my medical form could've been incorrectly filled out and they mentioned that I did have the vaccinations in the UK.

Odd.

11/23/2005 - Approved !

11/24/2005 - Visa arrived ! Whow.

12/02/2005 - POE Cincinnatti

12/27/2005 - SSN Applied for

12/31/2005 - Get married :)

01/03/2006 - SSN Arrives. Coor

02/02/2006 - AOS & EAD paperwork mailed

02/07/2006 - NOA1 for both above.

02/27/2006 - Biometrics, Columbus Ohio

04/11/2006 - RFE for AOS - Wife's US Citizenship details.

04/20/2006 - RFE info received and processed.

05/10/2006 - AOS RFE for last years tax info, due to AOS being processed after tax year end. Grrr.

06/07/2006 - 2145EST - EAD Approved. Whoo

06/08/2006 - Start job :)

06/10/2006 - Recieve EAD card.

07/02/2006 - Receive AOS interview date - 5th September

09/05/2006 - AOS Interview

09/07/2006 - AOS Approved. Email welcoming me to the US. Yey.

09/11/2006 - Receive the 'Welcome to the US Letter'. Green card is in the post etc.

09/18/2006 - Receive 2 year conditional green card.

I have seen similar posts about other areas of the country. I suggest that if you do have to drive a long distance that you call around first and ask lots of questions. When I started looking for one around my area I got a wide range of costs. I ended up paying about $100 but some were as much as $500. I also recommend you take the xray from the visa physical with you, the surgeon that saw my wife wanted to take another xray so I whipped out the one she got in Manila and saved myself $35. Thank you guys for your advice and info.

I just figure since Hicham's sister has already sent us the vaccination sheet from Morocco that 10$ and a half hour drive wouldn't hurt in order to try to avoid an RFE even though it seems like it'####### or miss with the RFE's for medical exams. Hopefully the vaccination supplement will be enough for them!
