marriage name on forms

We have almost completed our forms. The question we have is does the middle name effect things? Recently married female's name has changed.

Here is an example of pre-marriage name that shows on Passport, visa, etc.

Last name, first name, middle name

Chang, Van, Thi Hong

After marriage:

Smith, Van

Question is: can newly married person fill out current paper work with new married name of Smith, Van or is she required to keep middle name...so currently married name would be Chang, Van, Thi Hong?

I don't believe she can omit her middle name without a court ordered name change. The marriage cert will work for the last name. The exception would be if the middle name is part of a family name under a different naming convention, but the way you describe, it is a middle name. He doesn't want to put her middle name on the forms and is wondering if the marriage is a good enough reason to stop using the middle name, since the last name will change.

You want to put every detail exactly as it is, just in case something doesn't add up. They perform background checks etc so it's probably best to put all of your information just to be safe...

My marriage certificate has a section that says, "New names (if any)". If yours doesn't, then it is a bit more ambiguous. Did you get married in the states? Then you should see what the laws of your state say about name changes.
