Form I-134

I have a doubt (or 3 doubts), may someone help me?

1 - on I-134 Form question #7: "I derive an annual income of...".My fiance is not self-employed.

Should he put his annual income from 2005 or 2006?

2 - Does he have to send 2 copies of form I-134?

3 - Should he send "original" documents:

-proof of current employment

-individual Federal income tax returns for the most recent 3 years

-W-2s for any of the most recent 3 tax years

-Forms 1040

-copies of last 3 US Federal Income Taxes (form 1040)

-copy of his birth certificate

-new letter of intent

and their copies?

Lisa, the only thing I didn't mention on the PM when answering your questions you was this: he'll need the latest federal tax returns (not for the last 3 years) - form 1040.

Also, from the form:

Evidence should consist of copies of any or all of the following documentation listed below that are applicable to your situation.

This applies to the supporting documents (as well as proof of citizenship).

The sponsor must submit in duplicate evidence of income

and resources, as appropriate: ...

1 - his annual income as of today, on an annualized basis (current rate of pay x hours or pay-periods per year).

2 - how many people are applying for visas? You alone, or are you bringing a child with you? Some consulates require 1 per visa appliant, others require 1 per family unit applying at the same time.

3 - 1 photocopy and the 'original' in case the consular officer wants to verify the photocopy. Recognize that some 'original' documents are the same thing as a photocopy, e.g. paystubs printed from an online payroll service, in which case it's a moot issue. Recognize also that the original of a tax return is in the hands of the IRS. Common sense (often an oxymoron) is called for.

I don't have kids, so only I am applying for visa.

and what about birth certificate? I am afraid if he sends me original by mail, it might get lost.

Should he notarize his birth certificate's copy?

As far as I know, a copy is fine, although it's better be safe than sorry and get it notarized. But also, in US they can order birth certificates as original copies, paying at an office of registration for that, it would be the same as going to our cartório and getting an original of our birth certificate every time we needed it, meaning he can send an original and still have the original back home.

Nowhere in the instructions they ask for notarized copies of any documents to be gathered (not in packet 3 or 4 or on I-134). If they wanted notarized copies, the instructions would say so clearly so there wouldn't be room for doubt.
