I have the list of forms that the Embassy says Ian has to fill out and return. On the list, the DS-156K is crossed out. Does Ian not have to fill that out???? I appreciate all responses. Thank you in advance.

Also, is he suppose to send in copies of birth certificates and marriage certificates? The instructions on what to send in to them are a bit confusing. On the checklist it doesnt say to send it in, but on some parts...it says it does. What do we need to send back????? PLEASE HELP!!!

DS-156K is for fiance visa. Thats why its crossed out. Here's what else he should take with him:

Marriage cert.

His Birth cert.

Divorce certs. (If any, for both of you)

Death certs. of previous spouses (if any, for both of you)

Police reports from places lived since age 16.

Affidavit of support. I-134 and related documents (Your job letter, bank statements etc.)

DS-156 (2 copies)

DS-230 only Part I.

You can email embassy so they can answer your questions.

I will let someone else going through same consulate as yours answer more precisely.

In the UK, you send back the forms (omitting the DS156K) straight away, including the checklist which has a list of the documents.

It is quuite clear (in mine anyway!) that you should NOT send any documents with your forms. I will be sending mine momentarily, after I have booked a medical and filled out the date in there, so I will let you know if I run into problems! But I have been reading this site for weeks and I'm pretty sure I've got there in the end :)
