Hello

I completed my Medical at the time of visa process and Dr informed me that i have to take Varicella 2nd dose after a month so i am in US now do i still need to fill the form ? or get the vaccination done from the physician and give them the certificate that i completed my vaccinations

Suggestions please

Your shots are not complete because you still need the second varicella dose. You need to see a CS, get the shot, have them transcribe your DS-3025 PLUS the new shot into the I-693 and you'll be fine.

You fill out part 1 only of the I-693. Do not sign it or date it. Then take I-693 to your vaccination appointment together with your proof of prior vaccinations. The civil surgeon will fill in the other parts of the I-693.

You fill out part 1 only of the I-693. Do not sign it or date it. Then take I-693 to your vaccination appointment together with your proof of prior vaccinations. The civil surgeon will fill in the other parts of the I-693.

My fiance had vaccination appointment today, and they give her required shots, but did not give her or request any paperwork. They did however, ask her to sign some papers after appointment and shots, but that was all.

I'm a little confused. Your time line says you are still on a K-1 journey, not AOS. I was not required to do a I-693 when I was on my K-1 journey. Maybe Columbia is different. If Columbia does require a I-693, I can't help you there since it was not part my K-1 process. Sorry.

My fiance had vaccination appointment today, and they give her required shots, but did not give her or request any paperwork. They did however, ask her to sign some papers after appointment and shots, but that was all.

This post is in the AOS forum. The I-693 is used when filing for AOS after your fiancee already received the K-1, entered the US and married you. This does not apply for the medical that is required for the K-1 interview.
