Hello All!

My fiance and her son received their form DS-3025 during the medical appointment. However after reviewing it I noticed the doctor checked the Vaccine incomplete box and the Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above). Will we need to complete form I-693 with a civil surgeon?

I was also wondering if we should fill out all her forms for AOS, advance parole, work authorization, etc. in her new married name? If I fill out form I-485, should I complete Part 6 "Signature of person preparing form, if other than above"?

If you do not use her married name, her documents (GC etc) will not be in her married name they will be in her maiden name.

I had my wife (then Fiancee) and my stepdaughter go to the civil surgeon. It seemed like a good investment of $100. just to be sure. If you do, the doctor will seal it, don't open it. BTW, the DS-3025 my wife and stepdaughter had the same incorrect "vaccine incomplete" box checked. The civil surgeon said it happens all the time.

I pondered this question myself and decided for us it was easier to leave everything in her existing name ... it's completely up to you, no right or wrong.

If all the required vaccines are listed on the DS-3025, then you can just send a copy of that instead of getting an I-693. However, if either of them needs more vaccinations, you need to find a civil surgeon that will not insist on a full medical to have the vaccinations transcribed to the I-693. You only need Part 1, Part 2-vaccinations section only, and Part 5 filled out. Make sure to insist you do not need another medical, and even bring the I-693 instructions to prove it. It should only cost you around $50(or less) + any needed vaccines.

If she is planning on taking your last name, then you should fill out all the forms in her new married name. Her EAD/AP and green card will come with whatever name you use on the documents when filing. It is a long wait, and costly, to try and change the green card later.
