INS number is fine- it's essentially the same thing, they just changed the name a few years back.

Do you have your old greencard? It should have the "resident since" date on it. which is most likely the date you arrived in the USA. If not, try to remember the month at least.

Just use a plain piece of white paper and continue your list of family members, after stating at the top what this list pertains to (ie "Question #- list of Family Members continued:)

Not sure how Q14 is worded. If you cannot put ASAP on it, maybe put a date a month or two out? Or check with the embassy when you can get an interview for the SB-1 and put a date a couple of weeks after that.

Best to use standard letter sized paper as that is what they are used to in the USA, but I used A4 in Ireland it it was fine.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.
