Medical Exam..what to bring

My husband is about to do his medical and I read somewhere that he should bring 2 photographs,his passport and of course, the form for the doctor.But I also recall seeing something about him being required to bring proof of immunization..??The only immunization he has had is yellow fever as he has the card,so wouldnt the DOCTOR administer whatever other vaccinations he will need to come to the states??I'm confused..Thanks.

If the doesn't have proof of all the immunization required by US-CDC for immigration, he's going to have to prepare to have his arms pricked a little. The doctor will say he needs this, that, and those shots. And he's going to have to get those.

If he hasn't had any of the necessary immunisations then he should receive them at the medical. Proof of immunisation is only necessary when those immunisations have actually been received beforehand, in which case one needs proof in order to avoid paying for and receiving medically unnecessary second injections :-)

He will require the following as I have just done my medical in London...not sure if its the same all over, but....;

1 passport size photo, london accepts uk size

passport prefered I.D

Copy of police cert or original if still in his possesion

vaccination sheet from his doctors if obtained or else he will need it faxing from his GP to the panel doctors

Glases if worn. I took my reading glasses but was not asked about close objects or reading with them

If on medication, needs to take it with him

and prefilled form IV 18a which is available from the panel doctors site assuming it is required at the one he will attend

and any paperwork of on going medical conditions
