The online DS-156 form is now updated to its new version.

Expires: 6/30/2014

I was able to print one with a barcode today.

Hi. I tried this updated form but it shows only the answers I made. The questions in each item don't show up. However there is a barcode. What does this mean? Is this fine?

Anyway, I still have the expired one with me if the above won't be printed what it should be.

To Rara&Chym...happened to me too.. its 4 times before I can get it.. just try to click the back and forward then print and if not still working back it again and forward that is what I did..goodluck :thumbs:

Thank you for the reply RenCor. I tried back and forward but when I backward it, the form is blank and have to fill it up again. I tried it 3tims already. :(

Hey, that is not okay :) It happened to me too. What I did was refresh my browser until the questions show up. If it doesn't, try to restart your pc, empty your browser cache, then fill up the form again... if it still doesn't display right, refresh the browser again until the pdf is correctly displayed.. I did this a couple of times today... I used Mozilla and IE7...

You might also wanna try another browser like Google Chrome or Opera ~ see if it works.
