Alright.

So far AOS has us so frustrated it's unreal. I'm going to skip everything to this point and just be very succinct. We received an RFE. It asks for us to submit "Form 1-693" because "Part 5 is blank".

I had my medical in Vancouver in August. They took those forms at my POE. When I got down here to GA, I had a civil surgeon do the supplementary form for the immunizations. She signed them in part 6 or whatever that is (i'm not too sure if that was right but I'm looking at my copy of the I-693) So I called them and told them I received this notice blah blah blah and they said they will not sign in Part 5 because that is for the civil surgeon.

Confused I called USCIS.

They told me my RFE is an error and to make an Infopass appt.

BEFORE I make my info pass appt... can anyone offer insight on anything here??

You said you had a "civil surgeon" do the supplementary form for the immunizations. Then you said they will not sign Part 5 because that is for a "civil surgeon"? That is contradictory. Did you get a civil surgeon listed on the USCIS website to fill in your I-693? The civil surgeons are listed here: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

Yeah, who did you go to for supplementary form? Sounds like they weren't a designated civil surgeon.

Sorry for the confusion.

I had one civil surgeon do my medical in Vancouver.

Then in Chattanooga I had another civil surgeon do the immu records... and they won't sign "part 5" because they did not do the medical. The one in Chatt has been doing these forms for a long time and they said this is the first time they've had an issue. :wacko:

I want to make sure this is an error... ATL has one day in July with appointments and my husband can't get time off that week due to holiday scheduling.. so we have to travel 5 hours to SC to see someone.....

The doctor doing your medical in Vancouver was panel physician.

No need to make an infopass, they can't help you with this because it's not an error on USCIS side.

From what I understand, the civil surgeon won't sign Part 5, unless they do a complete medical. You can go back and show them instructions for I-693 which state that a new medical is not necessary, only parts 1, 2 and 5 for K-1. If they still refuse to sign it, then you can repeat the medical with them or go to another civil surgeon who will do just the vaccination supplement and sign it properly.

If they signed part 6 then you didn't go to a civil surgeon - you went to the local health department. A certification from the health department is only required if the civil surgeon referred you to the health department because they found a potentially disqualifying health condition.

Yes, I'm sure your local health department fills out these forms all the time, but they don't sign section 5 which must be signed by a USCIS certified civil surgeon. Your I-693 is useless without a signature from a civil surgeon.

I suggest you use the link which dinya provided, find a USCIS certified civil surgeon in your area, and try again.

It's possible you may be getting jacked around by one of the many civil surgeons who refuse to sign the I-693 unless you pay them to perform the medical exam. If they've really been doing these forms for a long time then they know damn well they can transcribe the vaccinations and sign section 5. The form instructions are very clear about this.

A civil surgeon should NEVER sign part 6 of an I-693.

A civil surgeon should NEVER sign part 6 of an I-693.

I agree. I think they are one of the ones that refuse to fill out the I-693 without you paying for a full medical that you do not need. You can print out the I-693 instructions to show to them, and if they still refuse, then find another one that will do it correctly for you.
