Reference with I693

Fellow Friend,

My wife had to go through Immigration Medical for K1 Visa at US Consolute. We expected vaccinatioin worksheet would be just enough for Adjustment of Status. But, we got a reference today asking for I693 from MSC. Did any of you experience similar situation? Our local office is San Antonio'TX but she got the reference from MSC and asked to send I693 back in MSC.Please share your experience/thoughts Thanks.

We also received a RFE for another medical exam and the vaccination supplement. We did not send the vaccination supplement in with our AOS packet. We took My wifes work sheet to a qualified cival surgeon and he transcribed ( his nurse did) it over to the Supplement to the I693. I sent this in the sealed envelope along with a note stating My wife arrived on a K-1 visa and had her medical exam whith in 6 months of filing the AOS. 4 weeks later we had here AOS interview appointment letter for April 2nd.

Instructions on the USCIS website and the I-485 application form clearly state a vaccination supplement is required. The supplement is only available and to be completed by USCIS-designated Civil Surgeons.
