The vaccination supplement is when the CS completes the vaccination table in the I-693 and signs the form. They think you forgot the rest of the form :S

If your medical is still valid, and your shots are up to date you don't have to send anything. You CAN send a copy of your DS-3025 (and I did and would recommend it) to remind them your medical info is in there. This is what you SHOULD have done... now your have to fix it. Your best bet would be to see a CS and ask them for a vaccination transcription, AND include a copy of your DS-3025 as well as the I-693 instruction page that says K1's with a valid medical don't need a new one... and hope for the best.

It does NOT mention completing only part 1.. that's completely pointless :S

My medical was done October 2010 - so its still valid. As for a DS3025 - I cannot find it anywhere.

Its a copy of the vaccination sheet you should of gotten from the medical.

You take that with the vaccination portion of the I-693 to the civil surgeon. They transcribe the vaccinations and add any new ones you were to keep up with after entering the USA. They sign stamp and seal it and you put that with your AOS. Sounds like you did not properly complete the task.

I sent a copy of my vacinations from birth -> as current as possible. The Hep A&B was a 6 month course that I started when the initial k1 was sent in. I couldnt be more current if I tried. I also sent in the record of my flu shot.

You can't just send records of your vaccinations alone, they won't accept them. They have to be transcribed to I-693.

This RFE has nothing to do with the validity of your medical. They are asking for vaccination records which have to be documented on I-693.

Since you don't have DS-3025, your only option is to go to civil surgeon, bring your vaccination records and ask to have them transcribed. I don't think there's anything else you can do to fix this RFE.

Incorrect. If your shots are COMPLETE on the DS-3025 then there's no need to get a transcription done.

And yes the RFE DOES have the do with the validity of the medical. The OP sent in ONLY page 1 of the I-693.. forcing USCIS to think that they had forgotten the rest of the I-693 medical. That's the problem. USCIS is ignoring the K1 medical because they think the OP had another one done based on them sending page 1 of the I-693. The OP needs to either just do an i-693 medical, try and get a copy of their medical from their home country, or try sending a I-693 transcription with a letter of explanation of their K1 medical still being valid and *hope* USCIS accepts that.

OP didn't mention if vaccinations were completed on DS-3025. They sent in their own records of vaccinations so I assumed they weren't. In that case they would need to have them transcribed to I-693 instead of just sending in copies of their own records. Or just not send anything if vaccinations were completed on DS form.

OP keeps saying that their medical is still valid and what I was trying to say is that RFE has nothing to do with validity of the original medical exam. They only sent one page of I-693 so USCIS thinks they did the whole medical again. Therefore RFE is not about the K1 medical as they did not even look at it, thinking there was a new medical done.
