Vaccination Document given my Knightsbridge Dr's in UK

Hi Everyone,

Just a quick question and hoping for a little advice. After my medical in London, they gave me a vaccination document that states what vaccinations I have had and if any were missing. I am up to date, however, she told me that no one ever gets the box on the form ticked that states "Vaccine history complete for each vaccine, all requirements met". She told me why, however, I was so nervous at the time I can't remember exactly what she said. It had something to do with a vaccine that no one really has so they have to tick "vaccine history incomplete", but she said I don't need anything else. On my form the box that states " Applicant may be eligible for blanket waiver because vaccinations are not medically appropriate". So my question is.......I am currently filing out forms for AOS, do I need to fill out any additional forms for this or can I just send this form in because it asks for a waiver?

Yeah, the waiver means you basically are complete. Some of the vaccines you don't need because of your age, and some because you have had the disease already, and others aren't available.

The waiver doesn't mean you're complete, but it doesn't mean you're incomplete either. If you're incomplete they can tick that box if you're unable to have certain shots for whatever reason (pregnancy or whatever) but can also be because some shots are age inappropriate.

If your shots are complete then you can just go ahead and send a copy of the DS-3025 with the AOS, if your shots aren't complete then you will need a vaccination transcription and have the missing shots.
