Marrying US Gf in October, OPT grace period ends AUG 24 -> Questions I 485

Hi,

After having dated my GF for 2.5 years, we decided to tie the knot. We are getting married in october but I am already looking into filling out the required documentation.

Looking at the I485 I seem to have run into several snags:

1. I entered through Canada the last time i came to the USA and an US immigration officer inspected my passport there. He took my I94 i filled out and kept it. What to do now?

2. My opt grace period is ending AUG 24. I was supposed to go on a CAP_GAP since I have a h1b approved for october first. My job at that company however ended. They are not revoking the H1b, but, I guess its pretty worthless if i wont get a job there. (they are not revoking it so i can apply for other jobs there). WHAT DO I FILL IN UNDER CURRENT USCIS STATUS? OUT OF STATUS or F1? and what about the expiration data? my I94 from my second to last entry states D/S...

3. I need to write down the location of latest entry and it is in canada, however, it asks for US state. Should i write CHI, IL as I flew to O'hare?

No one can help me ??? I am getting pretty stressed...

Added to the list: I did not receive an I94 when I entered through montreal. Maybe i filled one out but the customs officer must have kept it. I did get my passport stamped. Should i pay 300 bucks to file for new I94?

Also, should my fiance, the USC, use her maiden name in the documents, or my last name.
