Keep in mind that no flu shot will be needed if the July date of the DS3025 is used, even if your application goes in late October. They go by the date on the DS3025 for determining "complete". But if you decide to do a new immunization record with a CS, then that new date will be used by USCIS for determining if she is "complete". If you see the CS after Oct 1 (flu season), then a flu shot will be needed, as well as, the CS may not sign off without additional shots in the MMR series for example. While the second MMR could not be given in July, it could be given after time had passed. So new form with CS may mean new requirements and additional costs.

Are second-series or booster shots a good idea from a medical standpoint?

If so, we might do it at our family doctor's or local health department, and it would have nothing to do with the USCIS or CS. Just curious as to whether these shots are medically beneficial. If there are little to no benefits, I would rather my fiancee not have to endure another round of shots. My reasoning is, she didn't need these shots if she remains in China. If it's only required for immigration purposes, then I would skip them.

chicken pox in adults is dangerous and usually called "shingles". Chicken pox can also be deadly to babies. I personally think that it would be responsible to be immunised against it so that she's covered for not only her health, but the health of people she is around. I've heard of "chicken pox parties" to expose their kids to chicken pox all at the same time so they get it as kids (not babies) when it's relatively safe - if a little annoying - rather than waiting till their older when it's dangerous or having the shots. I remember having chicken pox as a kid, SO itchy! I stand by the statement that you should see a CS but Nich-Nick has often quoted from the USCIS manual saying that they go by the date of the medical. However I recall reading you need to be current "at the time of filing". Worst case scenario you don't get it done, you get an RFE, then you get it done. No drama :) Not the end of the world.

I had chicken pox when I was a child too :). No, it wasn't fun, but fortunately I remember very little of it, possibly because I was too young to remember anything at the time, and it's been a long time ago. :D

I asked my fiancee if she had it, and she thinks she had it too. Her's must not be itchy enough for her to remember well :D

Yes, prevention is good, and we'll make sure she gets all the care she needs in addition to immunization when she gets here.

And I agree that RFE is not the end of the world, especially for AOS, since she'll be with me, so it's zero drama compared to a k-1 RFE, thank goodness! :D

Any ol' DS-3025 may not be acceptable. They'll give you a DS-3025 whether you got the required shots needed for AOS or not. Some doctors fail to sign them. Some fail to complete the results section. I will say Knightsbridge clinic is very good at ticking the right boxes and signing if you got your shots and told them you had chickenpox.

There are certain things your DS-3025 should have to be considered complete.

Look at your DS-3025. If it has the following items, you don't need an I-693.

1. Shots marked (at least one of a series)

  • MMR
  • Td or Tdap or DT or DTP or DtaP (date no longer than 10 years ago)
  • Varicella or VH written by it if you had chickenpox

2. Everything else marked not age appropriate or medically appropriate.

3. RESULTS section filled in with "incomplete". AND "may be eligible for blanket waiver"

4. Signed and dated by the doctor

If it's not completed like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025.

Do NOT do what UK person ECWilloughby did recently and send in an I-693 without a doctor's part filled in along with the DS-3025. Do one form or the other.

The USCIS is supposed to have your medical files, but sometimes lose them or fail to match them up with your AOS. Do not get another exam unless you get an RFE that says they do not have results of your medical. It means they lost them, but you are probably stuck with paying for a new exam. Your official vaccination results are also part of that file. We send the DS-3025 photocopy more to remind the person opening the mail that we are intending to use that DS3025 to prove our vaccinations, rather than an I-693. The official "sealed" version is in you A-file (brown envelope) which they are supposed to get and match up with your AOS.

The DS3025 has to have all those things, not just some of them. Incomplete is fine if the box about blanket waiver is also ticked.

We are about to start the AOS package and we are also very confused on the whole matter with the I-693 form. I got my medical done on the 21 of June, 2011, and i am looking at the DS 3025 right now to see if everything is as u have described.

DTP - checked, and it also says completed series (same goes for TD, Polio-OPV, Rubella, MMR) on the other ones it says either ""Not Routinely available" or "Not age appropriate" or "Not REquired"

Results - Vaccine History Incomplete" "applicant may be eligble for blanket waiver"

Signed and Dated by panel physician.

Does this mean its complete or we still need the I-693 form for the AOS?

I don't know. It's the "Not Routinely available" that is your problem. I'm guessing that's varicella. If you had chickenpox as many in the UK have (person I was responding to) then the line would be marked VH for "varicella history". The doctors in London take your word for it. Your form without varicella or VH isn't complete as described.

Because as a K1, you are in the US with an opportunity to to get a varicalla shot, the "not available" isn't an excuse now. You can get one before applying for AOS. I have seen CR1s have shots waived for not being available, but they are essentially approved for a greencard before entering the US and can't get one before the approval decision is made.

I've never seen anything in writing that addresses "not routinely available" so it could go either way. Maybe they would glance right over that and waive the shot. You could send the DS-3025 and see what happens. If you get an RFE, then go for the shot and find a civil surgeon to fill out an I-693. That may cost you 2 weeks time on the greencard approval. If your current record is accepted, it could save you 100 bucks. Just pick the one that appeals to you most. Neither choice is the end of the world.

We took our records and used a CS in US to make sure everything was complete, he did blood test, TB filled in paperwork, no problems. Received a sealed envelope with gold seal and records for us.

The only question I have is regarding "Insufficient Time Interval". I have it checked off for DTap (last date is 4/7/09) and Influenza (last date is 11/22/12). Other than that, my DS-3025 meets all the requirements.

Oh, I also noticed Alien (Case) Number was never filled in by the doctor. Would that pose any issue?

I was in the same case, no alien number (it's not a problem), and "Insufficient Time Interval" for DTap. The doctor also wrote "next booster in 2018", so nothing I could do until 2018. I sent a copy of my DS-3025 in my package, got no problem.

I was in the same case, no alien number (it's not a problem), and "Insufficient Time Interval" for DTap. The doctor also wrote "next booster in 2018", so nothing I could do until 2018. I sent a copy of my DS-3025 in my package, got no problem.

Hi there i wanted yo ask u guys if my Ds-3025 says at the results box : "applicant does not meet vaccination requirements for one or more vaccines and no waiver is requested " can i just send that document in ? it's flu season and i haven't got a flu shot - it is marked in my sheet as not age apropriate ? and also for the Td it says insuficient time interval .

Can i just send it or should i go see a civil surgeon and get it transcripted ?

I'm thinking it's because of the flu shot that the dr wrote i dont meet the vaccination requirments ? what do u think ?
