hi..im new here...just noticed a part of that form omb no.1651-0111 is still stapled/attached to my passport...i left the US on nov 16, 2010. and ive read at the back that this must be surrended. pls help what am going to do..tnx and more power

hi everyone...im new here and i want to ask this question. i just noticed theres a piece of paper stapled/attched to my passport and its a part of form OMB 1651-0111. i read the back and it has to be surrendered. i left the US last nov. 16, 2010. what will i do with this since i read the back that failure to do so may delay entry to US in the future and am planning to see the US in summer this year.. tnx so much for early reply and more power

seems its I-94 that you have still attached on your passport.

Airline personal should had taken it when you boarded the flight, since you should have a immigration date stamp which can prove you left the country in time.

I think there is an address where you can mail the i-94, but not very sure of that.
