we are in the process of filing my AOS... i just have questions...

1. do i really have fill up the form I-693 even if i had my medical in SLMEC last March 31, 2011 then attach the DS 3025?

2. or i will just put my DS 3025 as attachment of I-485?

You can certainly try that option - others have done so and been successful (I was one of them) but then others have not been successful and have received RFE's (Request for Evidence) asking for the Vaccination component of the I-693 completed by a civil surgeon. If all of your vaccinations were complete at the time of your overseas medical and you have a copy of that vaccination worksheet showing this (DS-3025), you could try to see if they will accept just that documentation instead of getting the I-693 Vaccination section done. If they have a problem with it, they will send you an RFE for it and you will have to get it done and returned to them, thus causing a delay in your processing. Sometimes they do accept the worksheet only and sometimes they don't. There doesn't seem to be a way to predict one way or the other, although recent history has been that they are sending RFEs for the I-693 Vaccination component even if the DS-3025 showing completed vaccination history was submitted.

If you needed to get new vaccinations after your arrived in the States then you absolutely do need to use a Civil Surgeon and complete the vaccination section of the I-693 (and the personal information section, as well, of course). Many Civil Surgeons will try to tell you that you need a complete medical - you don't. Stick to your guns on this one and if the Civil Surgeon you contact won't give you just the vaccination part then find another one who will. The price for this will vary from $35 to $100 or so - there is no set price and each CS can charge what they want.

Another thing to consider is sometimes USCIS seems to lose the submitted medicals and will send an RFE for the whole I-693. While it really sucks big time if this happens it is better to redo the medical and re-submit it. Some who have challenged the statement and said that they didn't need to redo the medical as it was in their initial package have found their AOS' denied and have had to go through the process of getting it re-opened, then getting the medical redone and submitted anyway - so hopefully that won't happen in your circumstances, but if it does, you will know what to expect.

SLMEC does not offer at least one of the required vaccinations (TD if I remember correctly). You will need the I-693

Agreed. They did not give the me TDAP in Manila and I had to get it here.. Do the I-693
