2 Questions..I-129f and Letter of Intent

Hello all,

We are still in the process of gathering documents to submit our petition for fiance visa. My first question is regarding the I-129F. I'm looking on my birth certificate and on the I-129f its asking for the certificate number and where it was issued. Well I see a "state file number" and a 8 digit number on the bottom left hand corner, neither says the exacts words "certificate number" not sure which number goes on the I-129F?

My second question is regarding my fiancee's letter of intent. At the top she just has her full name, no address typed underneath her name and at the bottom of the letter she typed her name and then signed. On the example that I was using. I actually typed my name and address at the top. I also put the address of the Dallas Lock Box, below that It states, United States Department of State, United States Consulate...I added Philippines. Below that I added the address of the US Embassy in the Philippines because I figure that's where it will ultimately end up. I'm concerned about hers not having an address and I'm also concerned her name is typed not printed. Is this an issue?

I'm not even sure if I have filled mine correctly by adding the country of the Philippines or the US Embassy's address in the Philippines?

I hope I've typed this clear enough for you to understand what I'm asking.

That part of the form confuses a lot of people.

If you were born in the US then just check the box for "Birth in the U.S.", and leave everything else in question 10 blank. You've answered the question.

If you are a naturalized US citizen then check the box for "Naturalization", and provide your naturalization certificate number in the box immediately below. Leave the rest of question 10 blank.

If your citizenship was derived from your parents (e.g., born abroad with at least one US citizen parent) then check the "Parents" box. If you ever got a certificate of citizenship (many US citizens born abroad did not) then check "Yes", and provide the certificate number in the box immediately below. Otherwise, check "No". Leave the rest of question 10 blank.

The address is not relevant. The only thing that matters is that it contains the statement of intent and a signature. A date would also be good.
