Your case is transiting through National Visa Center (NVC) and will be transferred to Montreal's consulate. You can start calling NVC to obtain your local case number. That's what you will need to track your case once it makes it to Montreal.

Expect your case to sit at NVC for 2-3 before it makes it to your local consulate. Packet 3 will be mailed 3-4 weeks after the consulate gets your case. Montreal is notoriously backlogged and slow. You will likely not receive your Packet 3 for another 5-6 weeks (sorry!).

Good thing is, once your case is transferred to Montreal, you can call Department of State (DOS) to get updates. Montreal doesn't take calls and rarely answers emails. Once DOS tells you Packet 3 has been mailed to you, you can send the consulate your package. If you wait for your actual letter to arrive by snail mail, it could make you wait for another extra month.

Everything you will need to provide is listed here : http://montreal.usconsulate.gov/content/content.asp?section=visas&document=index

Good luck!
