@staceluvyou- do you know when they were actually upgraded? i called DOS and my packet 4 was sent out this Monday. Wondered/Hoping this upgrade was AFTER June 6 so we don't have to do the paper work over.

So does anyone know, it appears that the only choices are between an expired form or one that doesn't have the proper barcode? Does no one know how to complete a up-to-date barcoded form?

when you first click on the DS 156 its the old form but once you fill it out and click continue on the next page you will have the corrected up to date form filled out along with the barcode.

Hm, nothing is working for me...When I go to the DS-156, fill it out and hit continue it is still the old one...I-134, I have deleted the old ones on my computer and downloaded it again..same thing...old form

In fact, I-134 didn't change but expiration date is 05/31/2012 now.

Has anybody been able to download DS-156 with new expiration date? (That including barcode once is printed out)

I tried to do it on Department of state webpage and keep on showing the old one...
