Hello All,

Here is some background on my situation. My father is applying for his green card through my sister (his daughter) who is a U.S. Citizen over the age of 21. However, on my sister's birth certificate there are a few of errors:

1) Our mother's maiden is Seo, but on the birth certificate, it is spelled Soo.

2) When my sister was born, my father was 35 years old, but on the birth certificate he is listed as 37.

3) Both middle names for our parents are not included, even though they are going to be included in the application.

Unfortunately, we do not have time to amend the birth certificate.

My questions are:

1) It seems like I can include a One and the Same Affidavit for our mother. eg. http://www.legalwebreview.com/affidavits/affidavit-form-show-person-mentioned/ Is this correct? b) Should I include my mother's passport or consular card in my father's application to verify that her maiden name is really Seo?

2) Should I include an affidavit correcting our father's age. If so, how should that affidavit look like?

3) Will it be a problem that my parents' middle names are not included? If so, how should I solve deal this issue?
