I didn't fill it out, I had a search on google and my impression is that it depends which country you come from as to who fills it out.

You could do it to be on the safe side I suppose.

Brazilian beneficiaries, for example, are asked to fill it out regardless of gender.

DS-157 is required for males between the ages of 16 and 45, optional at the discretion of the consular officer for others. Many consulates, but not all, exercise their discretionary power to require DS-157 for women also.
