AOS - DS 3025

So we got our marriage certificates after a little less than a month of waiting, which isn't too bad, considering they said 6 weeks!

Anyway, now I'm ready to file for AOS... and I have everything ready and signed, and going to mail it tomorrow. But then a thought occurred to me after reading through the AOS forum. I did my medical in Toronto and received a copy of the Immunization form, where it says I'm completely immunized for age appropriate shots, etc... and the check boxes, etc..... but I don't think it actually says DS 3025 on it.

I know in theory I don't need a new medical since I had mine done less than 6 months ago, and I planned to send a copy of the receipt with the date of my medical (indicating it was less than a year ago), as well as the Immunization records I received from my medical.

I guess my question is, is the paper to which I am referring the DS 3025 that needs to be sent? Or do I actually need to go to a Civil Surgeon in California to get a new immunization records sheet filled out?

I'm kind of annoyed with myself for not thinking of this sooner :angry:

Also, I posted this in the Canadian forum, rather than the AOS forum, to see if anyone had similar experiences with Toronto or Montreal. :)

May 31st, 2011: Filed AOS

June 6th, 2011: NoA1

June 13th, 2011: Received Notice for Biometrics

July 7th, 2011: Biometrics

August 22, 2011: AOS Interview - Approved!

August 29th, 2011: Greencard in hand!

When I filed for AOS..I did not get my medical transcribed, I just sent in my medical worksheet they provided me from the medical in Toronto. In the Packet I included a letter stating when I had my medical done and that I have had one done in the last year, Stated where I got it done with the address and phone numbers listed. Also, I printed out the instruction sheet from the I 485 packet regarding the medical and highlighted it..lol Just to be safe I also photocopied my reciept from dr. Seidens office as well as the chest x-ray result he gave back to me. They had no problems with it and I got no RFE's. Some people get there medicals transcribed once in the states before filing for AOS, but I wasn't wanting to spend more money on it if I didn't have to, thought i'd wait for a RFE if that was the case, but it wasn't and we ended up saving some money :D Best of luck to you on the AOS process!!

Just to add..the form they gave me did not say DS 3025 on it...I forget what it exactly said, but it had all the lists of what shots I had and the doctors signature on the bottom with the checkmark boxes.

Gave the vaccination sheet that Montreal Medisys gave me. Had no problems but I did get the 693 transcribed just incase of an RFE. Never had a RFE tho.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

J&J, thanks for your info! It seems we received the same medical sheet, and I'll send it in. If I get an RFE for a new medical, I'll cough up the money for it then, rather than (hopefully) unnecessarily do it now.

Dr. Seiden's office gave me my chest xray as the actual xray rather than a CD as I've heard other offices do. Unless the Officer is also a radiologist, sending the xray is useless since he or she can't read anything from it. I hope they don't ask for it, but I guess if they do, I have it.

Inky - thanks for the info as well. There's about 10 Civil Surgeons in my area so I'll just wait for an RFE if I get one - I don't want to spend any more on this process than we already have!!

They won't want your xray, I just sent in the paper result, but they won't need that either!

I have my CD sitting here on my desk from my Xray. The DR here does not want it neither did USCIS lol..

My transcription only cost 37$ was really a drop in the bucket for a worry stricken self at the time!

I had the same question as you when applying for AOS, and decided just to send in the copy of the vaccination worksheet + receipt from Toronto medical Toronto medical (which apparently is identical to the DS-3025, but does not say DS-3025 anywhere on it), and deal with an RFE if I got one. I did not receive an RFE for transcription of the medical.

I tried calling a bunch of civil surgeons in my area, and spoke to some of their secretaries, and some of the physicians directly. Most of them refused to do just transcription of my vaccination worksheet from 3 months prior, and most of them tried to charge $300+ for an additional medical (no thanks!).

I even called the office in Toronto where I had my medical done, and they said they could not provide me with a completed DS-3025.

If you want to be on the safe side, there is a string on VJ about civil surgeons who will do just vaccination transcription for a reasonable fee. There may be one in your area.
