Help with DS-3032 - Choice of Address/Agent

Hi everyone,

I want to submit the DS-3032 by email but I cant seem to type

in the barcode. Deleting the "Place Case Barcode Strip Here"

doesn't work. So, how do I enter the barcode?

Also, what is the email address to submit the form?

You don't need a barcode when emailing it, you just type the info needed in the body of the email.
