Passport and Name Change application - Philippines

Hello, my wife's Filipino passport is expiring in July and we need to renew it. We plan on traveling to the embassy late next week, and want to make sure we have all of our stuff in order. We hadn't submitted anything about a name change due to our marriage in 2008, and I was wondering if both the marriage reporting form and the passport renewal can both be applied for during one day next week at the Embassy. I think I have the forms and such that we'll need to print out...just need that question answered.

Also, on the passport renewal/name change, since it is expiring soon, do I need to submit a Passport Amendment application or a regular passport application?

First thing u do is Report your marriage and once it is reported u can now renew her passport with the married name on it.

It can be done both at the same time.
