What forms do I need to file the AOS?

What evidence do I need to present, which would lessen the chance of an interview (would prefer not to travel for an interview)?

How many photographs are needed? Is it per form requirement?

Is one G-1145 required, or one for each application?

Which address do I send them too for New York State?

You need to read the guides and the form instructions

You do NOT supply any relationship evidence with the initial filing (other than marriage certificate) so there is nothing to do to "lessen" the chance of interview

YMMV
