Notatize in the US or in Vietnam

I'm unsure about what the consulate asked.

"Beneficiary: In a signed, sworn, notarized statement, please list all of your relatives in the United State. Please include their full American and Vietnamese names, dates of birth, their relationship to you, and their current addresses and telephone numbers (home, work and cell) with evidence of residence at that address (such as utility bills and/ or driver’s lisence copies.) "

Does this mean that my fiancee get all the relative information (utility bills, name & date of birth, phone number) then she bring these information and sworn that the information is correct? Or each individual relative of her who provided information get their information notarized and send them to my fiancee?

4th visit,December 26, 2010

INTERVIEWED May 21, 2011 --> RE: Additional documents

5th visit, both of us went in to submit additional documents on June 16, 2011---> Result AP.

6th visit, I went to the Consulate on December 28, 2011

Got back to the US and asked Congressman for help.

March 10, Asked Senators for help.

March 27, 2012 DoS status indicated an expedite on my case after Senators inquiry

End of March, Phone call from Consulate at 11:30PM.

April 13 (or so) received letter asked to update medical and police report

Received VISA April 28, 2012
