Choice Of Address and Agent

I just recieved a email from the nvc stating choice of address and agent and there is a form for me to fill out with the info of whoever i pick to be the one to recieve mailing about my immigrant visa the question i have is that do i have to print this email out and then fill it out and mail it in or my wife the (PITIONER) could fax it to me and i could fill it out and send it back to her so she could mail it from inside the U S because the mailing system in my country is not the best thank you for your input in advance..And also if there is some way i could do it email that would be nice just please explain to how to do so...

Don't mess with the template--just print the form, fill in the blanks, sign it, scan it and send back to NVC via email, and if you do it that way it DOES NOT have to be sent from the beneficiaries account, but obviously they have to sign it.
