2 questions regarding my child and and tax form 4506T

Timeline

Alright, so my daughter was born in DR, is 3 years old and has never left the country. For the record she is my bio daughter and my spouse and I were married prior to her birth. I was told here that I don't need an I-130 for her and to go to the Embassy with whatever form and documentation etc. and they'd issue a US passport in about 6-8 weeks and would cost around $180.00ish.

What form do I need?

Secondly, I'm about ready to do my 864 but have to get something finished up with my tax lady first. She wants to know if I'll need to sign a 4506T form.

Also, What are the poverty income levels for 2008-09-10?

CRBA and a US passport form, plus the needed documents of proof. The USC will need to be on the birth cert or a DNA test will be needed.

oh yeah, the CRoBA form. Just finished reading it all and forms etc. but didn't see anything about a DNA test.

Have to prove the child is that of a US citizens to claim US citizenship. Without the USC being on the birth certificate, how else would one prove that it is their blood/biological child? DNA test.

Wow. DNA test it is. But where do you specifically read, "Have to prove the child is that of a US citizens to claim US citizenship. Without the USC being on the birth certificate" etc?

Anyone savvy on form 4506T? I didn't see anything about it here or the I-864 but I thought it's better to ask than not. Heck I'll just bring a copy of the 864 to for her to check out.
