I just sent in a copy of my husband's DS-3025 that basically said the same thing as yours did Coconuts... and we didn't get an RFE..... I was under the impression that was all we needed.

I think I will just send the DS-3025 and maybe see a civil surgeon in the meantime in case I get an RFE. That way I can send it out sooner.

Did they give you the second MMR shot right there? How long did the blood work take for the chicken pox test?

the blood work took like 2 days and i just had to go down the hall for the second MMR you need one after 18 apperntly, from what i understand it depends on who you go to too....idk lol and im just going off what i have seen to do on here. it would be nice if they would give us better instructions

Got our biometrics letter today, December 9th :) My birthday is the 16th, so I was hoping to have it done before then as a present :D

When did you get your forms for the I-693? Mine are dated as of Oct 11 2011 so they must be a new version. I was so confused with people telling me about part 5. On my form, Part 1 is your info, part 2 is a listing of dates for original medical and any follow up dates, and part 5 is a referral to Health department... The Civil surgeon certification is on the front page on mine and is part 3.

i did the vacnation section only, if you call around you can find a place that just dose that section i think its number 1,2 and 5, they may give you a hard time bc this is the only visa where you dont do it all at once. i read on here that you do need it, im not sure, but its worth not getting a RFE to me lol. I had to get a blood test for the chicken pox, my word wasnt good enough and a second MMR shot, you need two down here. insurance coverd both. the only thing i paid for was 80 bucks for the medical it self. they didnt fill out section 2 in toronto thats why they gave you the sheet for when you got here

11. I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

Nice! I must admit I'd like to work too. Still haven't received my NOA which doesn't surprise me...everything else I've done in this process has taken twice as long as others.

You should be getting your card in the mail really soon!! It seems to be taking about a week from what I've seen (it took a week for my husband to get his)

I have a teeny question... does the EAD expire when your I-94 expires or when your visa expires? I'm wondering because I'm filing my AOS after the expiration date on my I-194 and I can't figure out the EAD instructions and whether or not I should bother with the EAD form.

The EAD will have an expiration date one year from the date of it's issue, but from what I've read here, once you get your Green Card it invalidates your EAD (you won't need it then anyway) but it could be a month or two between the time you get your EAD and when you get your Green Card, and seeing as how it doesn't cost anything when you file for it with your AOS... why not?? Guess it depends if and when you will want to be able to work.

Oh, thank you! For some reason I thought it expired much earlier. It's definitely worth it then, thank you! I guess it's the same story with AP too then? In the off chance I have a reason to travel to Canada before my green card arrives, it might be handy to have AP...
