Ok, so when we are filling out I-693, do we use our maiden or our married name? My medical was done in my maiden name, does that matter? I am filing the rest of AOS in my married name so I assume everything will be done in my married name, I just want to double check.

Mine said that my vaccinations were not medically appropriate. I am not sure if that means I need the I-693 or not. I know Jeannie did hers. I don't know if mine said that because of the age restrictions (I didn't have all of them but many were not in my age bracket basically), and I didn't have the flu shot since it was out of season. Since I am adjusting status in the fall I assumed I needed to prove I had it (and I will be mad if I don't because it gave me a nasty reaction...apparently I am allergic! Had my first US hospital experience about 2 weeks after moving here...fun!)

I also thought we had to get them transcribed? Has anyone had the blanket waiver box marked, and didn't get them transcribed, and got approved???

Mine says that the vaccinations I'm missing are not medically required. I thought I'd read of many people just sending in the DS-3025 and having no issues. Now I'm confused!

I've had the flu shot at the medical (they made me, ugh!), the only things I'm missing on the sheet are HPV and "Zoster" . There are little stars beside those saying not required. Everything else is checked as complete or not age appropriate.

I could be wrong! I am really hoping someone can give us an answer lol! I don't want to have to find a civil surgeon and deal with all that if it is not necessary...

The fees! No wonder so many people immigrate illegally. And the daunting paperwork...ugh! I got word that they got my AOS pkg and I should receive word in 7 - 10 days. Sure didn't take them that long to cash our check (notice, American spelling) though!!

Ugh the fees. That's the main reason our AOS was so delayed, as soon as we had the money we had to replace the radiator in the car!

And can I just say the I-864 is easily the most confusing thing we've filled out in this entire process? At least, I think so!

i did the vacnation section only, if you call around you can find a place that just dose that section i think its number 1,2 and 5, they may give you a hard time bc this is the only visa where you dont do it all at once. i read on here that you do need it, im not sure, but its worth not getting a RFE to me lol. I had to get a blood test for the chicken pox, my word wasnt good enough and a second MMR shot, you need two down here. insurance coverd both. the only thing i paid for was 80 bucks for the medical it self. they didnt fill out section 2 in toronto thats why they gave you the sheet for when you got here

11. I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

I'm so confused. :lol:

I have no other vaccinations to get. I've received everything except what's "not age appropriate" .. I even got the second MMR shot. What do I need to do? I was under the impression that my vaccination history and DS-3025 would be fine, but now it won't be?

My doctor messed up my vaccination card :( I have a feeling they will make me do that test to find my vaccinations. UUUGH frustrating! I'll use it with that sheet at the civil surgeon and see if that works...but I never got a second MMR. I wasn't aware we needed a second one. Mind you, I had like 4 or 5 as a child? Something like that anyway.

I talked to someone here who got an RFE because they filled out the I-693...they saw that and sent her the letter even though they could have totally ignored the form. I didn't do one, so keeping my fingers crossed!

Ralyse : Yes!! Jeff looked at the I-864 (I gave that form to him to complete <evil laugh>) and wondered why he needed to even bother. Seriously, can't departments share information? sheesh

How do you get an RFE for that? Maybe they missed something on it?
