Mom-in-law to try for 3rd time to get visa

I am sure their process is not something they want published. I do not know what algorithms they use.

Just from observations of my own experience, others I know and what I have seen posted. The latter you never really know the full facts, so not relying on one response, just general trends.

1. It is not random.

2. In most cases the result is foreseeable.

3. There is the grey area in the middle and the odd perplexing case going either way.

For 90% plus there is no need for the 'interview'. Would save a lot of time and expense for all parties to do it by mail.

This is correct. Twice when my MIL was denied for a visa they simply handed the denial letter to her, stood up, and walked off. Each time the letter says "Not enough ties to home country", and the applicant is not given the chance to question the decision.

Check with yout MIL - everytime she got rejected, did CO already have a rejection letter in the passport even before CO came to window?

I often thought for most applications they could just mail an approval or a denial. I guess there is a Regulation that says you have to be interviewed. I know a friend of mine, 12 ish years ago, got a B2 by mail.

They used to do for mail for certain ppl like repeat application, over 60 years of age application. They have stopped doing it after 9/11 they want each and every person to be there in person.

He would have been 45ish at the time.

The thing is it is not really an interview, more like buying a train ticket.
