Immunization record from Ukrainian clinic

NVC requirements state that the immunization record should be issued by the clinic on the paper of the clinic's letterhead. What if the particular clinic does not have their own letterhead blanks? Is it sufficient to for them to just list all of the shots on a paper with a clinic's seal and signature of the responsible person?

My wife tells me that they were saying something about a special blank obtained in the Kyiv clinic that conducts the medical for the visa that is used instead. Anybody has any information on that?

When we did K1 for my fiancee, now husband last year in Kiev he took his vaccinations records ( this was a hand written paper, without any letterhead)to his medical exam. Then the doctor there transcribed all his vaccinations to an official form, they gave my husband a copy of this form. We use that copy to complete his vaccinations here in USA, he had 2 of them missing. When you file for AOS you need to have the vaccinations requirements completed and with this form you know if some are missing.

His original handwritten vaccination records were accepted at the clinic in Kiev without problems, seems thats the rule.
