I am about to submit AOS package. My fiancee had her Medical in Russia. However, during Medical they forgot to ask her for vaccination record, thus there is no vaccination certification from Medical. Thus, she brought vaccination record that is translated and notarized. I am assuming I need to fill-out vaccination record part in the I-693 form. Is this right or there is another form? I am puzzled because vaccination part is for Civil Surgeon to fill out. Do I just fill it out instead of civil surgeon and leave the rest blank? I am also attaching vaccination record.

No no no, since you do not have the DS-3025 just go to a civil surgeon and bring her vaccination records for him to see. He will determine if she needs any or if it is complete. The doctor should be the one to fill up the I-693 and then seal it. You send the sealed copy together with your AOS packet. Unless you are a civil surgeon yourself then you cannot fill that up. My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

If you are applying for AOS then it is suggested to post in the AOS forum. This is not a K-1 visa process issue.

It does say in the instructions for I-693 that if Medical for K1 was done it does not need any additional Medical.

Yes, filling out the vaccination part is not another medical. I did not have another medical, but I did have my vaccination records recorded on the I-693 for $25 and signed by the civil surgeon. The xray, bloodworks etc need not be repeated
